Rider Emergency Contact Information

Please provide your emergency contact information. This is typically a spouse or family member that the ride leader could notify should you suffer an accident or other health related issue on a club ride. Please list one and ONLY one phone number and contact name; don’t list 911 as an emergency contact phone number, we will always call that in an emergency.
